Home sleep testing process:

1) Complete and submit request form and consent form. Addresses are necessary for delivery of the device. Payment cards are requested to be on file due to instances of significantly delayed returns.

2) Submit payment.

3) Sleep test will be shipped with pre-paid UPS return mailer and instructions for use. Instructions will also be emailed, along with tracking information.

4) Once sleep test is returned, data will be downloaded, reviewed, and reported. Data can only be physically downloaded from the device. Results may take up to 1 week for delivery via email.

5) Results will be emailed/faxed to you and your referral source. When results are emailed, a day and time for review of results can be scheduled.
